Understanding ADHD

What's Actually Happening in the Brain

          Most people are told that ADHD simply means having a short attention span, and the explanation often stops there. We approach it differently, because attention is only one part of the picture.

          ADHD is closely tied to dopamine, a neurotransmitter involved in motivation, reward, and the sense that something is worth doing. In an ADHD brain, dopamine signaling tends to be lower or less consistent than in a neurotypical brain. That helps explain why tasks that are interesting, urgent, or novel can hold strong attention, while tasks that are routine or delayed in reward can feel almost impossible to start or sustain—even when they matter.

          The prefrontal cortex plays an equally important role. This region supports planning, working memory, and impulse control, and in ADHD it often functions less consistently, especially during low stimulation or boredom. As a result, ADHD is less about a lack of attention and more about difficulty regulating it. The brain can focus very well—but it struggles to reliably direct that focus toward the “right” thing at the “right” time.

          This distinction matters clinically because it shapes how treatment is approached. Stimulant medications help by increasing dopamine and norepinephrine availability, which is why they can be so effective. But they primarily address signaling, not necessarily the factors that influence that signaling in the first place.

The prefrontal cortex is just as important. This is the part of the brain responsible for planning, working memory, and impulse control, and in ADHD it tends to be underactive relative to other brain regions, particularly during periods of boredom or low stimulation. That's why ADHD often looks less like an attention problem and more like a regulation problem. The brain isn't lacking the ability to focus; it's lacking the consistent internal signal that tells it a task deserves focus right now.

          In our practice, we also look upstream at contributors that can affect dopamine and prefrontal cortex function. Sleep quality, blood sugar stability, certain nutrient deficiencies, and gut health can all play a role. These aren’t just general wellness factors—they can meaningfully influence how ADHD symptoms present day to day.
